how do i get the upload files bit to work , created folder and enabled the check box but how do i get it in the form?

You mean file upload in aiContactSafe?
You don't need to create any folder unless you modify the path in the control panel of AiContactSafe.
To activate it in a form you just need to add a new field of the type 'File' in the fields window and activate it in the profile. By default you don't even need to do anything in profile. You will have to activate it only if you modified the fields used by the profile.
